Information for New Patients
We are currently accepting new patients.
We recommend that new patients download and fill out the New Patient Intake form before the first appointment. This greatly expedites the process and allows us to see you quicker when you arrive.
If you do not have access to a printer, please arrive fifteen minutes prior to your appointment and complete the forms in our lobby.
Initial Visit Checklist:
Bring a valid photo ID
Bring a Medical Insurance card if applicable
New patient Intake Forms filled out
Arrive 15 minutes earlier prior to first appointment.
Wear loose clothing that allows access to your arms.
Medical history is important. If you have diagnostic tests and reports from other practitioners, ask your primary physician to fax us the last 5 office visits if possible as well as any imaging studies (like MRI, X-ray, etc.) performed for your symptoms prior to first visit.
Regarding payment:
With Health Insurance: Receiving treatment by Uni Clinic, you have contractually agreed to pay for services rendered to you. If you have Health Insurance coverage, we will file your claim(s) to the health insurance carriers you provided us. We should keep your most current health insurance information in your chart. Your health insurance contract(s) are between you and your insurance carrier. Therefore, the primary responsibility of paying for services and providing follow-up communication with your health insurance carrier(s), belongs to you. If your health insurance rejects our claim for any reason beyond our control, you are financially responsible. If your health insurance coverage requires the insured to pay a deductible and percentage or a copay, these amounts will be due the day of service. We will try to give you an estimate of the amount you may owe before your visit. Payment can be made by cash, check, Visa, Mastercard, Discover Card, or American Express.
With Auto Insurance/Lien: Receiving treatment by Uni Clinic, you have contractually agreed to pay for services rendered to you. If you have auto-insurance coverage, we will file your claim(s) to the auto insurance carriers you provided to us. It is important to inform us your attorney’s information. If you were involved in an auto accident and have employed an attorney, you must let us know of any change in attorneys. We should keep most current auto insurance or attorney’s information in your chart. Your auto insurance contract(s) are between you and the insurance carrier. Therefore, the primary responsibility of paying for services and providing follow-up communication with your auto insurance carrier(s), belongs to you. If your auto insurance rejects our claim for any reason, you are financially responsible.
No Health Insurance: If you do not have health insurance, you will be required to pay for all services at the time they are received.
Uni Clinic offers a cash payment plan with discounted rates if the payment is made at the time of visit. We work with patients to make treatments affordable by offering installment plans, if you have a financial difficulty or hardship, so that patients can receive treatment in a timely manner.
